Our Designated Civil Surgeons are officially authorized to perform all required immigration medical examinations required for completion of Form I-693. Each evaluation includes a medical examination performed in accordance with the strict guidelines established by the Department of Homeland Security, U.S. Citizen and Immigration Services, and Centers for Disease Control. Our Designated Civil Surgeons will record all data and results on Form I-693 for proper submission to the USCIS.
Please note: X-ray fee is not included since it may not be required for all patients. In addition, if you require blood titers or immunizations, separate fees apply depending on the specific ones you need in order to pass the immigration medical examination.
Personal Physician Care’s team of Designated Civil Surgeons has been authorized by the U.S. Department of Homeland Security and the U.S. Citizenship and Immigration Services (“USCIS”) to perform the required medical examinations necessary for completion of Form I-693 (Report of Medical Examination and Vaccination Record). Our highly experienced and equally compassionate Designated Civil Surgeons expertly perform all necessary immigration-related medical examinations. Generally, USCIS requires that anyone filing for adjustment of status to that of a permanent resident must submit Form I-693 completed by a Designated Civil Surgeon.
Form I-693 is used to report the
results of the required medical examination to the USCIS. The
medical examination is required to establish that an individual seeking
adjustment of status is not inadmissible to the U.S. on public health grounds,
which are divided into four categories as follows: (1) Communicable
diseases of public health significance; (2) Lack of required vaccinations; (3)
Physical or mental disorders with harmful behavior; and (4) Drug abuse or
addition. Medical examination results are confidential and are used
only for immigration purposes.

Basic immigration physical examination and mental status evaluation—the U.S. Citizenship and Immigration Services medical requirements mandate that the Civil Surgeon’s examination must include a search for any indications of infectious diseases involving the entire body, which may include private areas of the body.
Blood Test—Applicants 15 years and older are required to have a
serologic blood test for HIV (AIDS) and Syphilis (RPR). Applicants younger than 15 years of age may
require HIV and/or Syphilis testing at the discretion of the attending Civil
Surgeon or if there is a reason to suspect the possibility of infection.
Tuberculosis Skin Test—Applicants 2 years and older are required to
have a tuberculin skin test (TST) performed.
This test requires a return visit to our offices within 48 to 72 hours
after administration. If the test
reading is greater than 5 mm of induration, a chest X-ray will be required.
Proof of Vaccinations or Blood Titer to Prove Immunity—Most adults require proof of vaccination for Tetanus-Diphtheria Td (within the past 5 years), Measles, Mumps, Rubella, and Varicella (Chicken Pox). A blood titer is a test that proves an individual has immunity or antibodies for a particular disease. We will review your vaccination history with you in order to determine whether you have all the necessary vaccinations required by Form I-693. Again, please remember to bring all vaccination records with you to your scheduled appointment for your immigration medical examination.
Who must submit Form I-693?
In general, all applicants filing for adjustment of status to that of a permanent resident must submit Form I-693 completed by a Designated Civil Surgeon.
May any doctor perform the required medical examination?
No, only a doctor who has been specially designated by the U.S.
Citizenship and Immigration Services (“USCIS”) as a Civil Surgeon may
perform the necessary medical examination.
The USCIS rejects any Form I-693 completed by a doctor who is not a
currently Designated Civil Surgeon.
What is a Civil Surgeon?
A Civil Surgeon is a doctor authorized by the USCIS to
perform medical examinations of aliens in the U.S. who are applying for
adjustment of status to permanent resident or who are required by the USCIS to
undergo a medical examination.
Who pays for the medical examination?
The applicant is responsible for paying all costs of the
medical examination, including the cost of any follow-up tests or treatment
that may be required. Payments are made
directly to the Civil Surgeon.
